Agape Center for Academic Excellence, Inc. $20,000
Challenge grant for the construction of an outdoor playground and learning center. Agape was founded in 1995. It is a Milwaukee parental choice school and a partner school with Milwaukee Public Schools. Its classes range from K4 through eighth grade.

Benedict Center for Criminal Justice $20,000
Support for the “Harm Reduction Program” to help participants live safer and healthier lives and reduce the harm that they do to themselves, their children and the community. Services include AODA counseling, case management, health services, HIV prevention education, anger and stress management, women sexuality. Programs aim to reduce prostitution among ex-offenders, improve parenting and provide support in a step-by-step approach.

Brady Area Foundation for Arts and Education, Inc. $25,000
Capital support for the Pulaski Street Playground. This restoration is a neighborhood initiative led by the East Village Association to completely revitalize a playground and park in this densely populated east side neighborhood. Existing facilities for tennis, baseball, and picnic area will be revitalized as well in this neighborhood north of Brady Street.

Catholic Charities of the Archdiocese of Milwaukee $500
Supports the expense of a dinner meeting with the speakers participating in the Catholic Charities Conference on Aging.

Catholic Charities of the Archdiocese of Milwaukee $20,000
Support for the Parish Social Ministry to Older Adults Program. This program targets parishes with large numbers of elderly parishioners and provides technical support to volunteers and professionals in designing and delivering services to the elderly. Seventy-five parishes have been identified and partnerships have been established with 25, with a goal of adding 12 additional parishes at the end of three years.

Center for Deaf-Blind Persons $25,000
Supports services to older adults with dual sensory loss. Services include rehabilitation training, self advocacy, employment and access to community support services. Outreach and case finding will be an important component of the program which will be operated in partnership with the Milwaukee County Department on Aging, the Interfaith Older Adult Program, the Milwaukee Housing Authority, S.E.T. Ministry and university and college interpreter training programs.

Central City Cyberschool $10,000
Support for the Library Book Fund of this new technology oriented charter school.

CEO II $25,000
Support for a new arts apprenticeship/jobs program initiative for Milwaukee area youth ages 14 to 21.

Children's Health Education Center $30,000
Support for expansion of an outreach program into middle and high schools. Programs are designed for youth in grades six through 10, and emphasize making positive health decisions, practicing nonviolence, resisting peer pressure and building healthy relationships.

Children's Service Society of Wisconsin $12,500
Support for a Child Assessment Center. This pilot project provides assessments and a plan of care for children entering the child welfare system. Children receive a range of services including medical, counseling, housing and education. If successful, this type of center will be replicated throughout the foster care system in Milwaukee County.

Community Advocates $20,000
Second-year matching support for the Health Advocacy Project to help low income families gain access to quality health care.

Community Memorial Foundation $20,000
Two-year grant in support of the Community Outreach Health Clinic. This primary care health clinic, which has been in operation since May of 2000, provides patient education and chronic disease management services within a free clinic serving Waukesha, Washington and Milwaukee county residents. Doctors and nurses volunteer their time and help patients with follow-up referrals and education, and management of chronic medical conditions.

Council on AODA of Washington County $25,000
Support for the Community Coalition Initiative. This project will build coalitions in the four communities of Germantown, West Bend, Hartford and Slinger to reduce youth substance abuse throughout the county. The Search Institute’s Youth Development Asset Model will be used to assemble the coalitions, train members, develop and implement action plans.

Creative SHARP Presentations $10,000
Second-year support for a program providing imaginative learning experiences using the visual arts to elementary school students in the greater Milwaukee area.

Donors Forum of Wisconsin $30,000
Two-year grant for operating support. DFW is the premiere Wisconsin organization advancing philanthropy through membership, professional education, collaboration, leadership and promotion.

Family House Clinic / Medical College of Wisconsin $20,000
Second-year matching support for this central city neighborhood health clinic operated in collaboration with the Medical College of Wisconsin.

Family Service of Milwaukee $50,000
Two-year grant to support a pilot project to provide integrated care coordination to elderly residents living in Section 8 apartment complex. If successful, this program will be expanded throughout the community. It provides low income residents with increased access to a full spectrum of physical and mental health and social services.

First Stage Children's Theater $5,000

Friends of Boerner Botanical Gardens $20,000
Support of the Horticultural Therapy Program for persons suffering from Alzheimer’s and institutionalized adults, for up to 120 participants per week.

Friends of Old World Wisconsin $25,000
Support of Project Innovation, a part of the 25th anniversary celebration of the Friends of Old World Wisconsin. This grant will support a revision of the educational curriculum used by the costume staff at Old World Wisconsin.

Future Milwaukee $15,000
Support for the Institute for Local Leaders. This program was established in 1993 to provide educational programming for newly elected officials and candidates for elected office. Courses cover campaigning, effective budgeting, handling the media, etc. Learning Circles incorporating peer-to-peer learning are a major element of this initiative.

Greater Milwaukee Foundation $5,000
Support for the publication of the year 2000 and 2001 Report Card on Charitable Giving. This annual publication of the foundation tracks charitable giving for 60 benchmark nonprofit organizations in the greater Milwaukee area.

Hebron House of Hospitality $15,000
Support for a Youth Services Coordinator to work with youth, ages six to 17 who use the agency’s emergency shelter, along with their parents. This program is a collaborative of the Donna Lexa Community Art Center and the nearby St. Matthias Episcopal Church. The program expects to follow youth, with their families, into permanent stable housing.

Hmong Educational Advancement, Inc. $15,000
Support for youth development efforts. Programming consists of a summer youth program and afterschool programs for approximately 100 youth each year. Homework assistance, tutorial support, cultural projects and structured recreational activities are included, as well as an Asian-youth gang diversion program.

Hope House $35,000
Support for a Community Resource Coordinator. The community coordinator will institute a volunteer management system; recruit, train and supervise volunteers and recruit new businesses and community organizations to support Hope House, a shelter/settlement house on Milwaukee’s south side.

Hunger Task Force of Milwaukee, Inc. $20,000
A two-year grant to support the procurement and distribution of fresh and frozen perishable food to the network of food pantries throughout the Milwaukee area. The program expects to expand from its 31 emergency feeding organizations to a network of 109 programs by the end of 2001.

Interfaith Caregiving Network of Waukesha $20,000
Two-year grant to support an orientation and training program for its volunteers who provide supportive services to older adults and adults with disabilities to help them remain independent in their homes. This new program will result in a curriculum and manual that may be used by other organizations in recruiting and training volunteers serving the elderly.

Jewish Family Services $15,000
Support for a collaboration with Catholic Charities, Lutheran Social Services, the Task Force on Family Violence and the International Institute to develop outreach and educational programs to prevent family violence in targeted refugee and immigrant communities in the Milwaukee area.

Leaders Forum $10,000
Two-year grant to provide fundraising and development skills to African-American Board Directors in the Milwaukee area. The course is offered over a nine-month period and trains approximately 20 individuals a year.

Leave A Legacy Wisconsin $15,000
Operating support for this marketing and public relations campaign which raises awareness of charitable estate planning and its benefits, and encourages more gifts in the Wisconsin community.

Marquette University $50,000
Support for the Nursing Redesign Initiative. This collaborative involving Aurora Health Care, Covenant Health Care, University of Wisconsin-Milwaukee School of Nursing and the Marquette University College of Nursing seeks to develop new models of nursing work as a major remedy for the emerging nursing shortage.

Marquette University/WI Geriatric Education Center $10,000
in collaboration with UWM Center on Aging and Human Services. This grant supports a Midwest Best Practices in Aging Conference to be held in Milwaukee in the Fall of 2001. This conference will include a virtual conference on the Internet for two weeks following the live conference. Registrants will be able to see each speaker read information presented and participate in ongoing sessions about each session with other online registrants, receive print handouts and overheads, view vendors and receive other conference materials.

Marva Collins School $20,000
Second-year support for a curricular enrichment program that provides morning nutritional snack breaks, psychological services and an arts program including music, taekwondo, dance and choir.

McBeath Community Partners Endowment Program $200,000
To fund matching endowment gifts raised in 2002 by the Milwaukee Public Library Foundation, Public Policy Forum, Second Harvest and United Community Center.

Support for the Center's senior programs.

Milwaukee Public Library Foundation $3,000
Bonus grant for gifts in excess of $10,000 contributed to the Milwaukee Public Library as part of the McBeath Community Partners Fund.

Milwaukee Public Library Foundation $25,000
Technical assistance grant to assist the Library Foundation in expanding its endowment fundraising as part of the McBeath Community Partners Endowment Program.

Milwaukee Public Schools Foundation $5,000
Support for the Save the Music Program.

Milwaukee Public Theatre $22,000
Support for the theatre’s “InterActors” program. This is a troupe of women over the age of 60 who use theatre, comedy and movement to entertain, educate and enlighten the community about aging issues of the elderly. With the Foundation’s grant, the group plans to expand the program by recruiting and training more women. Programs are performed in churches, social service agencies and at conferences, and expects to reach close to 10,000 individuals in 2001.

Modjeska Youth Theatre Company $10,000
Support for a summer youth production “The Me Nobody Knows.” Students from Milwaukee’s public central city high schools and middle schools actively participate in this effort. Typically, 40 students are on stage and many more are involved in behind-the-scenes activities including scenery, costumes, lighting, sound, publicity, music and stage management. The company tracks youth school attendance and requires a commitment to remain drug, alcohol and gang free. National research supports the conviction of the sponsors that theatre is a powerful medium for nurturing the growth and development of young people.

My Home, Your Home $10,000
General operating support for this shelter facility for children who have been removed from their homes and are awaiting foster care placement or return to their families.

Nonprofit Management Fund (Milwaukee) $70,000
Supports small technical assistance grants to improve the management of nonprofit organizations in the Milwaukee area. The program serves Milwaukee, Ozaukee and Washington counties and has 16 funding partners.

Nonprofit Management Fund (Waukesha) $20,000
Supports the small technical assistance grants program in Waukesha County, in partnership with 7 other funders.

Older Adult Service Provider Consortium $74,000
Three-year grant to implement a strategic plan to help create a formal nonprofit organization. The consortium has been in existence for over 15 years as an informal entity, and is now moving to establish a formal organization in light of the major statewide developments in the care of the elderly brought about by the state implementation of Family Care, the government system of paying for services to the elderly that will radically change organized services in the state.

Support of a fitness/wellness program designed by the UWM Department of Human Kinetics to assist persons with mental illness to integrate physical fitness and wellness activities into their recovery process.

Public Policy Forum $50,000
Two-year grant to fund a study of the Neighborhood Schools Initiative of Milwaukee Public Schools. This study will focus on what happens inside schools during the study period, and how each school uses the opportunities presented by geographic reform to promote better outcomes and greater parental satisfaction.

Public Policy Forum $5,000
Grant in recognition of the Forum’s success in raising five $10,000 gifts toward its endowment in 2000.

Riveredge Nature Center $20,000
Supports a partnership between Riveredge, UW-Extension and the Southeast Wisconsin Regional Planning Commission to educate high school students on the importance of land use planning for the long-term social, economic and ecological health of their communities. Approximately 500 students in ten high schools will participate.

Salvation Army $5,000
Support for the Christmas Tree of Lights campaign.

Second Harvest of Wisconsin $3,000
Grant in recognition of Second Harvest’s success in raising three $10,000 gifts towards its endowment during 2000.

South Shore Suburban Consortium $25,000
Support for the Certified Nursing Assistant training program provided by a collaborative of three southeast Milwaukee county school districts including Cudahy, Oak Creek and South Milwaukee. This CNA program provides students with entry-level skills in health occupations and assists area nursing homes and hospitals by creating qualified entry-level workers.

Special Needs Adoption Network $15,000
Second-year support for the Family Resource Center. This center aids families considering adoption and continues supportive services following adoption.

St. Ann Center for Intergenerational Care $15,000
Support of the Milwaukee Community Service Bank. This collaborative initiative was started in 1997 by St. Ann’s in conjunction with more than 40 providers of services to the elderly in Milwaukee County. Services are donated and are directed at the frail elderly on the county’s waiting list for the Family Care Program.

St. Vincent de Paul Society $15,000
Support for the Families Forward Program, which provides health, parenting, education and employment services aimed at strengthening families in the central city and northwest areas of Milwaukee.

Task Force on Family Violence $15,000
Support for a Children’s Advocacy Program designed to provide specialized case management and legal assistance services to protective parents whose children are victims of abuse.

The Red Bus Corporation $25,000
Operating support for a program that provides affordable transportation to recreation opportunities for residents of retirement and nursing homes.

United Community Center $5,000
Bonus grant for gifts contributed to United Community Center/McBeath Community Partners Fund in excess of $10,000.

United Lutheran Program for the Aging, Inc. $20,000
Support of ArtCare, an arts program for persons with Alzheimer’s disease. In addition to involving these patients in a variety of arts projects including dance and theater, the program will provide training to artists in how to work with dementia patients. This program is in its seventh year and has been a unique opportunity for family members to celebrate their loved ones as they are in the present and to cope with the loss of the familiar persons they knew prior to the illness.

University of Wisconsin School of Nursing $27,800
Support for a program to provide in-service training and consultation to home visitors who serve high-risk families caring for frail infants and young children. The training will provide three 4-day sessions and five follow-up consultations to 60 home visitors in small groups. This advanced training will recruit home visitors serving families in high-risk situations in targeted Milwaukee-area zip codes.

Urban Day School $25,000
Operating support for this private central-city school serving 900 students, kindergarten through eighth grade, on two campuses. Its programs include day care for children from six weeks of age, Head Start, afterschool and summer programs.

Urban Ecology Center $25,000
Support to develop an ecology playground at Riverside Park as part of the master plan to reclaim and rehabilitate the park designed in 1889 by Frederick Olmstead. It is located in one of the most densely populated neighborhoods in the state of Wisconsin and is bordered by the Milwaukee River. Surrounding neighborhoods and schools have been key players in this major restoration effort.

UWM/Helen Bader Institute $50,000
Core support for this new university institute which will provide nonprofit management education programming including credit/non-credit offerings; technical assistance related services to the nonprofit community; as well as collect, organize and disseminate relevant knowledge and research on the nonprofit sector.

Walker's Point Center for the Arts $20,000
Second-year support for the “Hands-On” afterschool arts program. This 13-year-old, southside community arts center provides free afterschool and summer arts programming in the visual arts for at-risk neighborhood children. Professional instruction is offered five afternoons a week during the school year and on days that the public school is not in session, as well as an annual arts camp offered from June through August. With an earlier Foundation grant, the program was expanded from three to five afternoons each week, and attendance is now at the capacity of the facility.

Wheelchair Recycling Program $40,000
Two-year grant in support of start-up expenses for operations in Milwaukee County. This program, begun in Madison in 1988, is expanding its services to Milwaukee County. The recycling program collects, cleans, refurbishes and distributes donated wheelchairs, scooters, walkers, canes, crutches and other medical equipment to individuals who need but cannot afford to purchase these items.

Wisconsin Coalition for Advocacy $20,000
Support for the Family Care Independent Advocacy Project. This project will monitor the new model of long-term care functioning as a state pilot project in Milwaukee County to redesign the long-term care services to the frail elderly. WCA plans to monitor the program and to provide advocacy support for those receiving care.

Wisconsin Conservatory of Music $20,000
Support for a Saturday Music Club for central city youth in the Parklawn Housing Development. This program expects to serve 60 children, grades three to five, over a three-year period. Students will be referred from the MPS CyberSchool and from the surrounding neighborhood. Music form, music history and appreciation will constitute the club’s curriculum. Emphasis on behavior, attendance and social skills will be part of the program and parents will be encouraged to participate. Organized musical presentations are planned, and student progress will be documented through videography.

Wisconsin Foundation for Independent Colleges $20,000
Two-year grant to assist with the design of a pre-college program to help Milwaukee central city youth complete high school and prepare for post-high-school educational opportunities.

WMC Foundation $5,000
Support for attendance of Milwaukee-area youth at the "Business World" program.

YMCA of Metropolitan Milwaukee $25,000
Support for the “Kids on Keys” piano instruction program at the John C. Cudahy Branch of the YMCA. This program will be offered in cooperation with the Wisconsin Conservatory of Music faculty to up to 60 youth to engage them in a year-round program of music instruction.

YMCA of Metropolitan Milwaukee $20,000
Support for the Career Life Skills Institute of the Youth Leadership Academy, a program recently affiliated with the Metro YMCA. The institute, designed for high school minority youth, incorporates job training, internships, personal development and entrepreneurial opportunities. In 2000, 42 youth participated. Of this group, 25 obtained private-sector internships at major corporations throughout the Milwaukee area. The YMCA expects to use this successful career life and skills model in its youth programs throughout the metro Milwaukee area.

YWCA of Greater Milwaukee $35,000
Support for the Girls Leadership Project. This program aims at closing a gap in many youth-serving programs by designing a curriculum specifically to meet the needs of girls.

YWCA of Waukesha $30,000
Support of a childcare program. This grant will help YWCA implement an initiative to position itself as a premiere provider of a continuum of childcare services in Waukesha County.
